What is Topotecan HCl?

Topotecan is a cancer (antineoplastic) medication. Topotecan interferes with the growth of cancer cells and slows their growth and spread in the body.

Topotecan is used in the treatment of cancers of the lung, ovary, and blood.

Topotecan may also be used for purposes other than those listed in this medication guide.

What should I do before use Topotecan HCl ?

Allergies? Tell your doctor if you have ever had any unusual or allergic reaction to topotecan. Also tell your health care professional if you are allergic to any other substances.

Pregnancy? Tell your doctor if you are pregnant. Studies in rats and rabbits have shown that topotecan causes birth defects and deaths of the fetus, as well as problems in the mother.

Be sure that you have discussed this with your doctor before starting treatment with this medicine. It is best to use birth control while you are receiving topotecan. Also, tell your doctor right away if you think you have become pregnant during treatment.

Breast-feeding? It is not known whether topotecan passes into breast milk. However, because this medicine may cause serious side effects, breast-feeding is generally not recommended while you are receiving it.

Children? Topotecan has been studied in a limited number of children. One study showed that seriously low blood counts may be more likely to occur in children than in adults.

Older adults? This medicine has been tested in elderly patients and has not been shown to cause different side effects or problems in older people than it does in younger adults.

 

Precautions of Topotecan HCl

It is very important that your doctor check your progress at regular visits to make sure that this medicine is working properly and to check for unwanted effects. Some of the side effects of this medicine do not have any symptoms and must be found with a blood test.

While you are being treated with topotecan, and after you stop treatment with it, do not have any immunizations (vaccinations) without your doctor's approval. Topotecan may lower your body's resistance, and there is a chance you might get the infection the immunization is meant to prevent. In addition, other persons living in your household should not take oral polio vaccine, since there is a chance they could pass the polio virus on to you. Also, avoid persons who have taken oral polio vaccine within the past several months. Do not get close to them and do not stay in the same room with them for very long. If you cannot take these precautions, you should consider wearing a protective face mask that covers the nose and mouth.

Topotecan can temporarily lower the number of white blood cells in your blood, increasing the chance of getting an infection. It can also lower the number of platelets, which are needed for proper blood clotting. If this occurs, there are certain precautions you can take, especially when your blood count is low, to reduce the risk of infection or bleeding:

-If you can, avoid people with infections. Check with your doctor immediately if you think you are getting an infection or if you get a fever or chills, cough or hoarseness, lower back or side pain, or painful or difficult urination.

-Check with your doctor immediately if you notice any unusual bleeding or bruising; black, tarry stools; blood in urine or stools; or pinpoint red spots on your skin.

-Be careful when using a regular toothbrush, dental floss, or toothpick. Your medical doctor, dentist, or nurse may recommend other ways to clean your teeth and gums. Also, check with your medical doctor before having any dental work done.

-Do not touch your eyes or the inside of your nose unless you have just washed your hands and have not touched anything else in the meantime.

-Be careful not to cut yourself when you are using sharp objects such as a safety razor or fingernail or toenail cutters.

-Avoid contact sports or other situations where bruising or injury could occur.

 

Topotecan HCl side effects

Each person's reaction to chemotherapy is unique. Some people have very few side effects, while others may experience more. The side effects described in this section will not affect everyone who is given topotecan, and may be different if you are having more than one chemotherapy drug.

We have outlined the most common side effects and those that are less common, so that you can be aware of them if they occur. However, we have not included those which are very rare and therefore extremely unlikely to affect you. If you notice any effects which you think may be due to the drug, but which are not listed here, please discuss them with your doctor or chemotherapy nurse.

Temporary reduction in the production of blood cells by the bone marrow. This can result in anemia leading to tiredness; an increased risk of bruising or bleeding and an increased risk of infection. The number of cells in your blood may begin to reduce from about seven days after the treatment has been given and usually reaches its lowest point at 10-14 days after the chemotherapy. Your blood cells will then increase steadily and will usually return to normal within 21-28 days. Your blood will be checked regularly to see how well your bone marrow is working.

If your temperature goes above 38C (100.5F); you develop any unexplained bruising or bleeding; or you suddenly feel unwell, even with a normal temperature, contact your doctor or the hospital straight away.

Feeling sick (nausea) or being sick This is usually mild and there are now very effective anti-sickness drugs to prevent or greatly reduce nausea and vomiting. If you do feel sick it may begin a few hours after the treatment is given and can last for up to a day. If the sickness is not controlled, or continues, tell your doctor, who can prescribe other anti-sickness drugs, which may be more effective.

Loss of appetite is also usually mild and may last a day or so. Cancer BACUP's information on diet and the cancer patient has tips on boosting your appetite, coping with eating difficulties and maintaining weight. The dietitian at the hospital can be very helpful.

Hair loss usually starts 3-4 weeks after the first dose of topotecan. Hair may be lost completely or may just thin. You may also have thinning and loss of eyelashes, eyebrows and other body hair. This is temporary and your hair will regrow once the treatment is finished.

Tiredness and a general feeling of weakness It is important to allow yourself plenty of time to rest.
